SAMPLE QUESTIONS
The CEPHAS Assessment uses three complementary question types across three spiritual domains (Love of God, Love of Neighbor, Love of Self) and nine “sub-scales (i.e. Prayer & Interior Life, Educational Witness & Ministry, Self-Awareness & Integration) to create a comprehensive profile of your spiritual formation journey. Our assessment investigates intellectual understanding (head), affective experience (heart), and readiness to grow (motivation). Below are sample questions that illustrate this holistic approach.
UNDERSTANDING (Head)
I have a clear understanding of core Catholic beliefs.
EXPERIENCE (Heart)
I feel God's presence with me throughout my day.
READINESS TO GROW (Motivation)
I've been considering ways I can deepen my personal prayer life.
EDUCATOR REPORT
The Educator report shares an overall CEPHAS score, scores in each of the three domains (Love of God, Love of Neighbor, and Love of Self), and an in-depth analysis of each of those domains, broken down into three subdomains each.
A formation journey progresses generally through five levels: Beginning, Developing, Growing, Thriving, and Flourishing. An educator can read personalized insights about their current levels within any domain or subdomain and view practical steps for continued growth through recommended practices and exercises.

*Please note that all Educator results, scores and formation levels, are private. None of the Educator’s individualized results are shared with their administrators. Administrators only have access to a school’s aggregated data.
